It is an oral factor Xa inhibitor that selectively blocks the active site of factor Xa and does not require cofactors (such as antithrombin III) to exert its activity. It is primarily used clinically to reduce the risk of stroke and systemic embolism in patients with non-valvular atrial fibrillation, to treat deep vein thrombosis (DVT) and pulmonary embolism (PE), and to prevent the risk of venous thrombosis following orthopaedic surgery.

Primary Indications

1. For the prevention of venous thromboembolism (VTE) in adult patients undergoing elective hip or knee replacement surgery.

2. For the treatment of deep vein thrombosis (DVT) and pulmonary embolism (PE) in adults; for reducing the risk of recurrent DVT and/or PE in patients with an ongoing risk of recurrence following completion of at least 6 months of initial treatment.

3. For the reduction of the risk of stroke and systemic embolism in adult patients with non-valvular atrial fibrillation who have one or more risk factors (e.g. congestive heart failure, hypertension, age ≥75 years, diabetes mellitus, history of stroke or transient ischaemic attack).

The Main Contraindication To Medication

1. Patients with known hypersensitivity to rivaroxaban or any excipients in the tablet formulation.

2. Patients with clinically significant active bleeding.

3. Conditions or lesions presenting a marked risk of major haemorrhage.

4. Concomitant therapy with any other anticoagulant is contraindicated, except when switching anticoagulant therapy or administering unfractionated heparin (UFH) at doses necessary to maintain patency of central venous or arterial catheters.

5. Patients with hepatic disease accompanied by coagulation abnormalities and clinically relevant bleeding risk, including those with Child-Pugh class B or C cirrhosis.
